Louis Malle has always has a penchant for dealing with touchy subjects - such as in Damage and Murmur of the Heart. In this movie I think he handles the subject of child prostitution in very realistic and delicate way. It is also a very interesting snap-shot of New Orleans in 1917. I believe the nudity and sexual situations add realism and potency to the film - although these days directors would not away with it (lots of things were gotten away with in 70s movies)
